APPENDIX A

FORMS

A number of the more usual and important forms required in lien proceedings by an architect may be helpful. The forms here given deal primarily with the early steps, preliminary to or upon the institution of the lien action. For other forms dealing with later proceedings, or forms less intimately connected with the rights of the architect, owner and contractor, reference may be made to any standard form books or special text books on lien litigation. The following forms are selected largely from forms used in actual litigation. They should, of course, be adapted to the special circumstances of the cases in which use may be made of them.

The labor performed and services rendered by the lienors consisted of services and advice rendered by them as architects. These included conferences, preparation and furnishing of various preliminary studies, working drawings, plans, large scale and full size detail drawings, and furnishing of specifications; conferences and advice reproposals and estimates and contracts; consideration of and issuance of certificates of payment, and the supervision of the construction work for and in connection with an apartment house building located on the premises hereinafter described and against which building, as well as against the said premises this lien is claimed; conferences, services and advice in connection with the letting and performance of the work under separate contracts; conferences and advice and extra and special services performed after the approval by the owner of said premises and by lienors' employer of the first definite scheme adopted and involving changes in and additions to various of the plans, drawings and details; preparation of entirely new and additional studies, sets of plans, drawings and detail drawings for the said building; applications to and conferences with the Tenement House Department and Bureau of Buildings in the City of New York; special attention to changes in floor plans, entrances, ornamental work and cellar layout and construction; conferences with respect to preparation of a renting plan and prospectus and advising with respect to same and preparing and furnishing the renting plan for such prospectus; conferences, advice and services on changes in plans and con
